Directions for Submitting a Fish Sample for Testing


If you are looking to submit a fish sample for testing, then you’ve come to the right place. Below are the forms and information needed to collect and submit your sample to the Aquatic Diagnostics Laboratory. All completed forms (below) must be submitted to Katie Bradshaw, ADL Administrative Coordinator, at kathrine.bradshaw@tamu.edu.

Work Order Form
This form is required before (email or fax) or at the time of sample delivery (hand deliver or in shipping package). Testing and diagnostics cannot be started until this form is completed. Failure to provide the Work Order form or incomplete forms will result in delays in the processing of samples. Please ensure that contact information is accurate. All completed forms must be submitted to Katie Bradshaw, ADL Administrative Coordinator, at kathrine.bradshaw@tamu.edu.

External Customer Information Form
This form is required for the new University payment system, and must be submitted before or at the time of sample delivery due to changes in University policy. Testing and diagnostics cannot be started until this form is completed. Failure to provide the External Customer Information form or incomplete forms will result in delays in the processing of samples. Select “Extension” when completing this form. All completed forms must be submitted to Katie Bradshaw, ADL Administrative Coordinator, at kathrine.bradshaw@tamu.edu.

How Much Will Lab Tests Cost?
The Aquatic Diagnostic Laboratory performs diagnostic tests at cost.  Associated fees recovers only costs for the labor and diagnostic materials, so there is no mark up to the client. Please visit this page for details regarding the current fees and forms of payments.
